The Binding of Isaac leaves players in control of a child protagonist named Isaac, trying to avoid death at the hands of his own mother, who believes she has heard the voice of God commanding her to sacrifice Isaac to prove her faith. Not wanting to die, Isaac finds a secret hatch and attempts to escape through the house's basement. He winds up confronting hordes of deranged creatures, lost brothers and sisters and his own fears along the way.

Throughout his journey, Isaac will find bizarre treasures that change his form, give him super-human abilities and allow him to fight off enemies - there are more than 100 power up items. Using his tears as weapons, Isaac can launch better attacks the more he cries. The Binding of Isaac also gives players items to use in-game, such as bombs, which cause damage to an area.

Massive amounts of randomly generated enemies and levels on top of items create tons of replayability alongside the mature themes of the Binding of Isaac. The use of cartoon style graphics combined with dark storytelling, dungeon structures inspired by old school Zelda games, and "roguelike" controls combine in order to create a strange but memorable gameplay experience.
